Access to Experts

Hospice Pharmacia’s Medication Management Support Centers provide our clients with ongoing, expert consultation. Our specialists collaborate with hospice nurses and (MMSC) prescribers to outline the most clinically effective, cost-efficient symptom management plan for each patient at the point of care. MMSC staff are accessible 24 hours a day, seven days a week, 365 days a year.

Access to Knowledge and Resources

Our clients have access to a wealth of palliative care knowledge and resources, including:

  • Our Medication Use Guidelines™ (MUGs®), which outline more than 50 evidence-based clinical protocols for palliative pharmaceutical care. Peer-reviewed and updated annually, the MUGs® help hospices provide consistent, standardized, cost-effective care.
  • HP Client Access, our on-line data management portal for nurses, physicians and pharmacists. Providing access to all hospice pharmacy functions and records, HP Client Access helps hospices streamline medication management. The portal also allows practitioners to share patient information, resulting in improved collaboration, and provides education resources and reporting tools.

Access to Medication

HP is a pioneer in providing unparalleled access to patient medication. The excelleRx pharmacy fulfills more than 18,000 prescriptions daily, and we are proud of our .04% error rate. We deliver via national overnight courier to a patient’s home or site of care.

  • Our HP RxCard gives hospice patients and their caregivers immediate access to medications through nearly 60,550 participating community-based pharmacies.
  • Mail order services provide convenient, cost-effective access to medications.

Custom compounds are available upon request, in the event that commercially available medications are not adequately controlling patient symptoms. Compounds are especially beneficial when alternative routes of administration are needed for medications.
