Hospice Pharmacia (HP) was founded in 1996, but our passion for serving hospice patients and their caregivers had already been growing for years before that. In the early 1980’s, what would later become Hospice Pharmacia began as a small independent pharmacy. This small pharmacy served a local hospice that was a participant in the original hospice Medicare Demonstration Project. Four other independent pharmacies soon joined the organization, and as the company grew, future HP leaders learned more about the special needs of the hospice population and discovered a way to partner with these hospices in a unique financial model.

As the partnership between this HP pharmacy and local hospices matured, we came to appreciate and develop the framework of what is now our shared risk model – the very philosophy that is the core of HP’s services today.

In a short time, this innovative service model was so in demand by local hospice communities that these few pharmacies recognized the great need to fill in the niche market of hospice patient care. Hospice Pharmacia incorporated in 1996 and eventually based operations in Pennsylvania and Tennessee.

What began as four independent, small pharmacies serving local hospices has grown to be the market leader in hospice pharmacy services. In subsequent years, the unique platform designed to provide care for hospice patients was recognized as equally valuable for other chronic diseases and patients states. To accommodate patient groups such as chronic pain and solid organ transplant care as well as an expanding expert medication distribution facility, the company excelleRx, was incorporated, with Hospice Pharmacia as its largest and core business. All the while, the company continues to operate on the same founding principles born years before and has now expanded to serve all venues of hospice care including long-term and inpatient hospice facilities. We've grown and learned from our hospice partners, instituting new services such as enhanced pharmacy benefits management (PBM) capabilities and customized pharmacy service solutions to meet changing industry needs.
