HP is proud to be the only hospice pharmacy service provider to offer wound care assessment, care planning, and specific products for the management of wounds at the end of life. Services include comprehensive, evidence-based, peer-reviewed HP WoundCare Guidelines; access to consultation with HP’s wound care certified specialists; as well as metrics and outcomes tracking. HP WoundCare consultation and the HPWC Guidelines are included in HP's per diem pricing. WoundCare products are offered to our clients under a fee-for-service plan, as part of our commitment to helping hospices manage their costs.
