HP IPU Services
 
The quality, efficiency, and standardization of medication management that Hospice Pharmacia (HP) provides for hospice patients in the homecare and long-term care settings is now available for patients residing in hospice inpatient units.

The cornerstone of Hospice Pharmacia’s comprehensive services is patient safety, which results from HP’s Prospective Medication Therapy Management system. This is coupled with HP’s secure, web-based patient care tool, Xeris®. Clinicians can view and print a complete Medication Administration Record (MAR), in real time, for each patient. The added feature of the remote dispensing cabinet enables on-site access to only those medications that the patient is taking to authorized users.


HP’s Service Options in the IPU Setting

HP works closely with hospices to determine the highest quality, and most cost-appropriate model to service your patients in the IPU setting. HP is positioned to support you through this process. When choosing a model for service with HP, the size of the hospice and state regulations that govern the hospice pertaining to remote dispensing should be the primary factors for consideration. The three inpatient unit service option models are as follows:

The Residential “Hospice House” Model
Similar to HP’s homecare model, patient-specific medications are provided by Hospice Pharmacia directly to the Hospice House. HP provides stock medication for the Hospice House.
The hospice and HP determine stock items and par levels. HP’s local pharmacy partners provide dispensing for urgent access to medications.

The HP Local Pharmacy Partnership Model
HP’s local pharmacy partner dispenses a 7-day supply of hospice/related medications. A hospice kit/emergency box is available for urgent symptom management needs. Non-palliative care medications may be provided outside the per diem by HP.

The IPU Model
Medications are stocked in the remote dispensing cabinet as well as in bulk storage/refrigerator. HP’s local pharmacy partner supplies all medications, including those for the unit. Medications that are not related to the primary diagnosis can also be supplied, upon request.

Quality

For hospices that choose to implement the XerisIPU model, HP authorizes the medications through Xeris that are required from the remote dispensing cabinet following consultation with the hospice IPU staff. Only the medications that the patient is taking are available to the authorized hospice user.

  • Remote Dispensing Unit, an optional feature that integrates with Xeris to provide secure, on-site access to medications at the IPU.
  • Local pharmacy partnerships provide support for routine and urgent medication needs, as well as inventory management.
