During the course of a patient’s stay on service, Hospice Pharmacia tracks clinical benchmarks and quality-of-life outcomes on a daily basis. The endpoints and outcomes tracked are specific for the patient’s admitting diagnosis. For instance, pertinent information for terminal cancer patients (approximately 40% of HP patients served) would include: level of pain, level of anxiety, level of constipation, level of nausea and vomiting, ability to interact with family, mood, etc. These endpoints and outcomes are available securely to authorized stakeholders via the on-line tool, Xeris.
