Founded in 1996, Hospice Pharmacia was the first pharmacy to focus on the expert management of medication for end-of-life patients. When the company expanded its services to include referrals from pain clinics and transplant centers, excelleRx was formed, with Hospice Pharmacia as its largest division. Today, HP remains focused on palliative pharmacy care for patients with a prognosis of fewer than six months.
