Hospice Pharmacia, a service of excelleRx, Inc., provides medication therapy management (MTM) for hospice programs throughout the United States.

Through highly skilled specialty pharmacists, evidence-based Medication Use Guidelines (MUGs), and state of the art technology, HP works directly with prescribers and nurses to ensure safe and effective medication use. We refer to our MTM model as “prospective”– meaning our pharmacists collaborate with clinicians at the point of care, helping to support the selection of the most appropriate medication, the first time. This reduces time-to-palliation of symptoms, enhances patient comfort and quality of life, and creates nurse efficiency.

Prospective Medication Therapy Management (PMTM) integrates prospective care planning and monitoring with interdisciplinary care management. PMTM reduces direct and indirect pharmacy costs by:
1) Identifying untreated symptoms,
2) Intervening when potential or actual adverse drug events are detected, and
3) Dispensing the most clinically effective medication.

Direct medication costs and indirect hospice costs, including unplanned after-hours nursing visits, emergency room services, hospital admissions, and long-term care facility admissions, are reduced through PMTM.
