Personal Information We Collect

When you visit excellerx.com, hospicepharmacia.com, excellerxinstitute.org, medrxperts.com or painrxperts.com, we do not collect any personal information without your consent. If you are perusing our sites and have not willfully submitted your personal information to us, none of your personal information is collected. On certain pages within our web sites, you are invited to register with us through the use of online forms and applications. In order to complete these forms, information such as name, address (including street name or post office box, city, state, and postal code), Social Security Number, and email address are required. We specifically reserve the right to access, monitor, and retrieve password-protected information to ensure your compliance with our Terms of Use and to comply with laws and regulations, such as HIPAA.

Additionally, composite tracking information generated by cookies for traffic accounting and content analysis may be shared with third parties for the purpose of documenting site activity and other trends. We emphasize that the foregoing statistical information is not linked to any personal information and cannot identify any individual user of our web site.

Our Retention of Personal User Data

Once you have registered with excelleRx, Inc., we will store and maintain the personal information you have volunteered for so long as you remain actively registered with excelleRx, Inc. or any of its affiliates and approximately 24 months thereafter (so long as you have not directed us otherwise), or for so long as we are required by law. Our retention of your data allows us to continue providing you with enhanced and personalized services without interruption.

excelleRx, Inc. believes that choice is one of the most important privacy issues. You have the choice not to volunteer your personal information to us. Access and Accuracy

excelleRx, Inc. aims to keep your personal information as accurate as possible. As an online user, we may contact you regarding any status changes in your personal information. To modify your contact information prior to our contacting you, you may send an email request to xerispw@excellerx.com specifying your status change. By doing so, we will add, delete, or change the data in your personal profile. Should you at any time desire to remove your name from our list of excellerx.com registrants, you may contact us at info@excellerx.com. After receipt of such a request, we will remove your personal information from our records as soon as commercially reasonable.

We also aim to maintain current and accurate health information records for the patients we serve. As an online user, you may want to update or modify a patient record in a field from which your access is read-only. You may update any patient record field by contacting one of our trained pharmacy professionals at our Pharmaceutical Care Support Center by calling toll-free 877-882-7820.
