Patient Reported Outcomes Measures (PROM) Program

PROM was created by Hospice Pharmacia in response to the challenges that hospices face in collecting patient-level data to meet the QAPI requirements in the Conditions of Participation. PROM is a data collection process that allows participating hospices to systematically capture and document patient symptom information. The data can be used to illustrate symptom outcome trends at the hospice level, and can be compared with other participating hospices. These comprehensive reports allow case managers to make “real time” adjustments in each patient’s care plan, and enable hospice QAPI teams to identify deficiencies and opportunities for improvement.

PROM is available as an à la carte service. The simple per-project fee is determined by the number of patients enrolled and the number of calls made.
