Hospice Pharmacia has been working closely with partnering hospices to identify their needs regarding the Quality Assessment and Performance Improvement (QAPI) section of the Medicare Conditions of Participation (CoPs). In response to input from our clients, HP has created an enhanced QAPI platform that helps hospices produce quality outcomes as well as satisfy many requirements of the Conditions of Participation.
Through HP’s opt-in program, Patient-Reported Outcomes Measures (PROM), data is collected for individual patients and combined to illustrate symptom outcome trends at the hospice level – providing hospices with a wealth of information through detailed reports. PROM participants are then able to compare their results to other hospices, giving them a competitive edge in the rapidly changing hospice environment while ensuring the best possible care for their patients.
