Medication reviews to identify ineffective medications, adverse effects, possible drug interactions, and more, are part of standard operating procedures at HP. Specially trained pharmacists review all medication care plans and use the annually updated Medication Use Guidelines (MUGs®) for an evidence-based approach to care that improves patient safety and outcomes.
Data collection and analysis has never been more important to hospices – especially now that Hospice Conditions of Participation (CoPs) include requirements for Quality Assurance and Performance Improvement (QAPI). Our enhanced QAPI platform helps hospices produce quality outcomes as well as satisfy the requirements of the CoPs.
Of course, we could never hold our clients to high standards of quality if we didn’t continuously strive to improve our own performance. In addition to establishing rigorous internal performance improvement goals, we conduct regular client satisfaction surveys and idea exchanges. In other words, we listen to our clients, and turn the results into better services.
