Reports available through Xeris, HP’s real-time web-based software, include:

Active Medication Report- Shows the active medication profile for all patients within a specific group(s) (can add inactive and discontinued medications) for a selected day.

Medication Summary Report- Shows the complete medication profile for all patients within a specific group(s) for a selected day.

Patient Profile Report- Shows profiled, dispensed, and discontinued medications for all patients within a specific group(s) for a selected day.

Antibiotic Report- Shows all antibiotics dispensed by HP or obtained through HPRxCard for a selected time period.

Assessment Batch Report- Shows all patient assessments completed within a specific group(s) for a selected day.

Billable Days Report- List of all patients that were active with HP, the number of billable days, and charge for those days for a selected time period.

Encounters by Nurse- Shows detail for all hospice encounters, including patient name, nurse name, and whether a PIN was entered, for a select time period.

LTC Authorization Summary Report- List of all medications authorized for LTC patients in a selected time period.

% Xeris Refills- % of refills done through Xeris compared to other ways (internally or through the ARDS) for a selected time period.

Local Pharmacy Report- Lists all medications dispensed by local pharmacies and the patient the medication was dispensed for, for a selected time period.

Quantity Dispensed- List of all medications dispensed in a selected time period and the quantity dispensed of each medication.

Hospice Analysis Workbooks- Compares hospice’s medication usage, delivery, % cancer, and ComfortPak usage for the entire year with the median of all HP partners.

Critical Endpoints and Outcomes- Compares different diagnoses of patients, with average score of assessment questions for the entire year, with all of HP’s partners.

% ComfortPak Acknowledged by Encounter- Lists encounters by patient and whether the need for placement of a ComfortPak was assessed for a selected time period.

New Admission Method- Lists the patients that were admitted or readmitted within a selected time period, and the method of admission (internal Xeris user vs. external Xeris user or through an interface).
