The Story
 
The company was founded in 1996.  excelleRx (then Hospice Pharmacia) was the first pharmacy to focus on the appropriate use of medication, to offer access to medication management information using a thin-client application service provider (Xeris), and to share-risk for the cost of medication using a per diem model.

excelleRx was created for one reason: to re-inject science into the medication selection and use process.  For the last five decades the selection of medication for an individual patient has been relegated to preference-based, pharmaceutical marketing-manipulated processes. This opinion-based prescribing has contributed to a chilling array of preventable medication-related problems, including incisive, well-publicized morbidity and mortality statistics, in addition to unrelenting waste in direct and indirect medication costs.

Since the founding of excelleRx in 1996, we have assembled an iconoclastic and determined team of Pharmacotherapy and PharmacoInformatics experts who have brashly engineered a novel, web-based, end-to-end medication management system.  The backbone of this precocious infrastructure is our enterprise software, Xeris™ (i.e., the Greek root word for knowledge).  Presenting patients, either individually or in large databases, are risk-stratified using our proprietary advisoRx ™ engine within Xeris, in order to quantify presenting perils of medication related problems. Advanced trained pharmacists analyze the results, patient by patient.  Applying peer-reviewed, algorithmic, evidence-based and iterative medication use guidelines, the pharmacists make specific regimen recommendations to prescribers, or, under collaborative practice agreements invoke the medication care plan, per se.  Longitudinal continuity is preserved by collecting Patient Reported Outcome Measures, directly from the patients or caregivers. 

This unique, pacesetter approach--risk-stratification, algorithmic customization, and integration of pertinent outcomes—to the medication selection and use process has resulted in successful positive outcomes in all three seminal areas: economic, clinical and humanistic.  The penultimate ‘tabula rasa’ opportunity for excelleRx is to diffuse this flourishing innovation upstream, migrating from our current focus on the hospice and palliative care market, to the frail elderly, and then onto all persons in need of an “air force” real-time center of pharmacotherapy excellence that will assist in navigating the perils of medication misadventuring.  Our ultimate opportunity is to further introduce into the medication selection process predictive models available to practitioners, patients and others at point of care—we call this our adamant goal (i.e., building and deploying) of launching a user-friendly, Predictive Pharmacotherapy Outcomes System.
