Soon, HP will be offering a hospice telehealth service for
Patient-Report Outcomes Monitoring, called PROM. Outbound
electronic contact to patients identified as appropriate
for follow-up has been found to enable nurses to sort patients
by symptom severity, prioritizing their day-to-day patient
care activities based on patient risk.
