Anticoagulation Risk Management & Monitoring

National Web-Teleconference for Hospice Partners

The Performance Improvement Team at Hospice Pharmacia will be presenting this educational training every Tuesday during the month of August.

Goals and Objectives

This training will help the learner be better prepared to:

- Identify best standards of practice for reducing the risk for errors and adverse events associated with anticoagulant use.

- Describe the Joint Commision National Patient Safety Goals and Elements of Performance as it relates to patient safety and anticoagulant therapy.

- Identify HP's policy and procedure on therapeutic monitoring of anticoagulant therapy.

- Identify the risks associated with anticoagulation therapy, including but not limited to warnings, contraindications, adverse effects, drug-drug interactions.
