Patient Reported Symptom Outcomes in Hospice: A QAPI Case Study

Call Description: This presentation describes HP’s Patient Reported Outcome Measures (PROM) program, in which symptom data are systematically collected, trended, and reported to hospice providers. Implications for practice and performance improvement are highlighted through PROM case studies and examples of project findings.

Objectives:

  1. Discuss challenges in collecting and reporting patient-reported symptom outcomes in hospice.
  2. Describe HP's quality assessment/performance improvement (QAPI) program PROM, in which "real time" symptom data are systematically collected, trended, and reported to hospice providers.
  3. Examine how partner hospices used PROM in their improvement efforts.
