Hospice Pharmacia is working with hospice software leaders to provide HP partners with the easiest, most efficient methods of communicating and updating patient information. XerisConnect is the electronic interface between HP’s systems and these hospice software applications. XerisConnect interfaces provide a timely, accurate method of sharing critical patient information. Unidirectional (ADT) interfaces send patient demographics (admission, discharge, transfer) from the hospice to HP. XerisConnect improves nurse efficiency by reducing the verbal transmission of patient demographic information and avoiding transcription errors. Future development includes interfaces for medication profiles.
